Who we are
Tomaszewski Practice is the data controller for the information described here. Our website is t-practice.com. You can reach us about anything on this page at tom@t-practice.com. We handle your information in line with the UK GDPR and the Data Protection Act 2018, and we follow the guidance of the Information Commissioner’s Office.
This website
The site has no comments, no accounts and no logins. We use no analytics, no advertising and no tracking cookies, and we load no fonts or scripts from other companies, so visiting these pages tells nobody else that you were here. Our hosting provider keeps ordinary server logs for security.
The Anxiety Plan runs entirely in your own browser. It asks for a username so that it can keep your place; what you write into it stays on your device and is not sent to us or to anyone else.
If you contact us
When you write or telephone to ask about an appointment we keep your name, your contact details and whatever you choose to tell us, so that we can reply and arrange to meet. If we do not go on to work together we delete that correspondence.
If we work together
Working as a psychotherapist means handling information about your health, which the law treats as a special category needing extra care. We hold: your name and contact details; the dates of our sessions and what you have paid; and clinical notes. Notes are handwritten and kept separately from anything that identifies you. We do not use your surname on invoices.
Our lawful basis for holding contact and payment details is that we need them to provide the service you have asked for and to meet our legal obligations, including tax. Our basis for holding clinical information is Article 9(2)(h) of the UK GDPR: the provision of health care and treatment by a professional bound by a duty of confidentiality.
Who else sees it
We do not sell, share or trade your information, and we do not use it to market anything to you.
Like every accredited psychotherapist, Tom discusses his clinical work with a supervisor. That discussion is anonymised: your name and anything that would identify you are left out.
Two things can override confidentiality. We may have to pass information on if a court orders it or the law otherwise requires it, and we may break confidence if we believe someone is at risk of serious harm. Wherever we can, we would talk to you first.
How long we keep it
Clinical records are kept for eight years after our last contact, following the retention periods in the NHS Records Management Code of Practice; for anyone seen under eighteen, until their twenty-fifth birthday, or eight years after the last contact if that is later. Financial records are kept for six years, as HMRC requires. After that, records are destroyed securely.
Your rights
You can ask us for a copy of what we hold about you, ask us to correct anything that is wrong, ask us to delete it, or ask us to restrict what we do with it. Some of these rights have limits where we are required to keep clinical or financial records. Write to tom@t-practice.com and we will answer within one month.
If you are unhappy with how we have handled your information you can complain to the Information Commissioner’s Office at ico.org.uk or on 0303 123 1113.
